Applicants may be requested to submit proof of eligibility with their application if their veteran is not a
member of Melvin Roads. Such proof can vary, but a common document is the DD 214. It is issued by the U.S.
government upon discharge from military service.
If you do not have a DD 214 here are some ways it can be obtained:
If you do not know the social security number, you may be able to obtain it on a death
certificate.
You may also be able to obtain the DD 214 from the American Legion or the VFW Post where
the veteran was a member.
